The idea here is to destroy as much property as possible within the time limit. The easiest way, I've found, is to take out rows of fences (if they are available in the zone you are in), which can really bring up your combo bonuses pretty fast. If you can't find a good row of fences, take out lamp posts and stop signs and fire hydrants and such. Personally, I like to stay in a decent car for these rather than getting out and shooting stuff. I just found it to be easier to mostly ignore the "bonus" items and stick with the easy to find props and rack up the combo bonuses.
Mayhem can be found in Nob Hill (Vice Kings), Southern Cross (Los Carnales), and Misty Lane (West Side Rollerz).
Rewards (completion of all 8 levels):
Los Carnales: Molotov cocktails unlocked in your weapon cache
West Side Rollerz: Pipebombs unlocked in your weapon cache
Vice Kings: Grenades unlocked in your weapon cache
All Three: RPG unlocked in your weapon cache
